Intake Forms

Intake forms collect important health history, preferences, and consent information from clients before their first appointment. BusyBook provides a digital intake system so clients can fill out forms on their own device — no paper needed.

Go to Clients > Forms & Intake to manage your intake form templates and review submissions.

This is where you create and edit intake form templates. Each template defines the questions your clients will answer. You can create different templates for different services or client types.

View all submitted intake forms in one place. Each submission shows the client name, submission date, and the template used. Click into a submission to see the full responses.

When a client needs to complete an intake form, you can share a link to the form. Clients open the link on their phone, tablet, or computer and fill it out at their convenience — in the waiting room, at home before their appointment, or anywhere with internet access.

The form is branded with your practice name, not “BusyBook.” Clients see your business name and a clean, mobile-friendly form.

  3. A submit button to send their responses

Once submitted, the responses appear on the client’s profile under the Intake tab and in the Forms & Intake submissions tracker.

There are two places to view a client’s intake responses:

  1. Client Profile > Intake tab — Shows all intake submissions for that specific client
  2. Clients > Forms & Intake > Submissions — Shows all submissions across all clients, with search and filter options

When you open a client’s profile before or during a session, the Intake tab gives you quick access to their health history, allergies, medications, areas of concern, and any other information they provided. This helps you tailor the session to their needs without asking the same questions every visit.
